How can someone's approval rating stay in the low teens for months and months and they still act like they're just doing the people's business.

I mean, just think about who comprises that 13%. Geriatric American Legion types, corporate lobbyists, country club types (Boehner's one real talent is golf), and insurance and energy industry executives (and other US Chamber of Commerce types). I'm not sure he gets many value voters (hey, he's at 13%!)since he doesn't mention abortion every third word. Maybe a few assorted teabagger types as well, but he doesn't have the right kind of Glenn Beck CRAZY going for him to be popular with that set.
